The 'chronic condition' cliff: exactly when your PMI stops paying — read from real Bupa and AXA claims wording
Every UK PMI policy pays for acute treatment and stops paying for chronic. Where that line falls — for diabetes, asthma, IBD or a stable back — is drawn in the small print. Here is what it actually says.
The short answer
- UK PMI covers acute conditions — those expected to respond quickly to treatment
- Chronic conditions — those requiring long-term monitoring — fall outside standard cover from the point of diagnosis
- The classification is made by the insurer at claim, not by the treating consultant
- The four most common surprise reclassifications are type 2 diabetes, asthma, IBD and mechanical back pain
The clause every UK insurer uses
Every UK PMI policy contains a clause of the form: 'We cover treatment for acute conditions. An acute condition is a medical condition of rapid onset that is expected to respond quickly to treatment. We do not cover treatment of chronic conditions.' Every word matters. 'Expected to respond' is the insurer's judgement at claim, not the treating consultant's opinion at diagnosis. This is the mechanism by which cover for a condition can be paid for the first three months of treatment and then declined from month four.

The four conditions that most often surprise members

- Type 2 diabetes: initial diagnosis and investigation is acute (covered). Ongoing management with metformin or insulin is chronic (not covered). Complications from diabetes (e.g. retinopathy) are treated as separate acute conditions and are usually covered.
- Asthma: an acute attack and inpatient admission is covered. Ongoing inhaler regime and pulmonology follow-up is not.
- IBD (Crohn's, ulcerative colitis): acute flare, inpatient admission and surgery are covered. Long-term biologic therapy is not.
- Mechanical back pain: initial imaging and up to 6–10 physio sessions are covered. Ongoing physio and pain management beyond that is not.
