UK private medical insurance is priced by full postcode. A 42-year-old on identical cover pays £1,842 in SW1, £1,376 in LS7. The seven pricing bands and how to sit in the cheaper one.
The postcode lottery inside your PMI: why the same policy costs 34% more in SW1 than in LS7
Every UK insurer prices PMI by full postcode, not by region. The same 42-year-old on the same policy pays 34% more in SW1 than in LS7 — and the mechanism has nothing to do with local health.
The short answer
- Every UK PMI insurer prices by full postcode, sorted into 5–8 hospital-cost bands
- SW1, W1, W8, NW3 and SE1 sit in the top band on all seven insurers
- A 42-year-old on comprehensive cover pays £1,842 in SW1 vs £1,376 in LS7 — same policy, same underwriting
- The mechanism is the local private-hospital tariff, not local health outcomes
Same policy, seven prices
We ran the same benchmark quote — 42-year-old non-smoker, comprehensive cover, £100 excess, mental health module included, six-week NHS wait option off — across the seven major UK insurers, in five UK postcodes. The spread was almost 34% between the highest and lowest, on identical cover. No underwriting difference, no age difference, no medical history difference. Postcode alone.

The mechanism — hospital tariff, not local health
This is the piece the industry does not advertise. Postcode loading is not driven by local disease prevalence, GP shortages or ambulance response times. It is driven almost entirely by the private-hospital tariff schedules that apply to that area. A hip replacement at HCA in central London costs the insurer 41% more than the same procedure at Spire Leeds. Insurers backload that cost into the individual policy through the postcode band. If you live in NW3, you are being priced as if you are likely to use HCA London — because statistically, you are.
"The cleanest illustration is a family that moves from SW11 to SW15 — same borough, three miles apart. Their renewal drops £180 on a comprehensive policy. Nothing about the family changed. The postcode dropped one band." — Tumaris, Director, PremierPMI
The three levers you actually have
- Choose a Guided or Directional variant. Both restrict you away from the most expensive London hospitals in exchange for a 12–22% premium cut. In a top-band postcode this often cancels out the postcode loading completely.
- Raise your excess. Moving from £100 to £500 knocks 9–14% off the premium at every insurer — a bigger single lever than most postcode moves.
- Cross-quote your postcode with your broker before renewal. Postcode bands are updated annually by each insurer, and a re-banding at renewal is one of the top-three sources of quiet premium creep.
What does not work
Registering an address outside your true residence to sit in a lower postcode band is fraud, and every insurer runs address-verification checks against the electoral roll and credit-file data at both application and claim. A single denied claim over an address discrepancy costs more than a decade of postcode loading.
Group and corporate schemes
SME group PMI is priced by the employer's registered office postcode across every member, which is why a Leeds-based firm employing London-resident staff often sees a significantly cheaper per-head premium than the same staff would get individually. This is the single most overlooked reason for very small companies (2–4 employees) to consider a group scheme even where individual policies would technically be simpler.
