The facts on IVF and UK private medical insurance

Fertility Investigation & IVF Cover UK

Infertility is usually investigated after two years of trying to conceive without success, or sooner if you're 36 or over or have a known risk factor ([NHS](https://www.nhs.uk/conditions/infertility/diagnosis/)). Comprehensive PMI typically funds specialist consultations, hormone tests, semen analysis and scans used to investigate the cause, but assisted conception treatment — IVF, ICSI, IUI and donor gametes — is excluded on every mainstream individual UK PMI policy; some employer group schemes add limited IVF cover separately.

How the NHS defines and investigates infertility

The NHS considers infertility when a couple haven't conceived after a year of regular unprotected sex, though more than 8 in 10 couples where the woman is under 40 conceive naturally within that time. Infertility is usually only formally diagnosed and treated after two years of trying (NHS — diagnosis; NHS — treatment).

A GP will discuss medical and sexual history for both partners before arranging initial tests. NICE's fertility guideline sets out reference values used to interpret semen analysis, including sperm concentration, motility and morphology thresholds, and states repeat testing should usually follow an abnormal first result (NICE NG257).

Treatment, where offered, can include fertility medicines, surgical procedures (such as fallopian tube surgery) or assisted conception, including IUI and IVF. What's available on the NHS depends on local integrated care board policy (NHS — treatment). This is not medical advice — always discuss your situation with a GP or fertility specialist.

What UK private medical insurance typically covers

Comprehensive PMI generally covers the investigation stage of fertility difficulty: specialist gynaecology or andrology consultations, hormone blood tests, semen analysis and scans, where referred by a specialist for a newly-raised issue.

Bupa's policy guide treats this diagnostic pathway as standard outpatient and specialist cover, subject to its chronic and pre-existing-condition rules (Bupa By You policy guide). AXA's Personal Health handbook similarly funds eligible outpatient specialist consultations and diagnostic tests, while separately and explicitly excluding infertility treatment (AXA Health handbook, April 2026). WPA's Complete Health IPID lists consultant consultations, diagnostic tests and complex scans as core in-patient, day-patient and outpatient benefits (WPA Complete Health IPID).

  • Specialist gynaecology or andrology consultations on referral
  • Hormone blood tests, semen analysis and pelvic imaging
  • Investigative day-patient procedures such as tubal patency testing, where eligible

What is excluded

Assisted conception treatment is excluded by name in AXA's membership handbook and is treated the same way across the UK individual PMI market: IVF, ICSI, IUI, ovulation induction for conception purposes and donor gametes are not covered, regardless of the underlying cause of infertility (AXA Health handbook, April 2026).

Related costs such as fertility medication for conception, cycle monitoring during treatment, and storage of eggs, sperm or embryos also fall outside individual PMI cover. This applies whatever underwriting basis the policy was sold on.

  • IVF, ICSI, IUI and donor-gamete treatment
  • Fertility medication prescribed specifically to achieve conception
  • Egg, sperm and embryo storage and related monitoring

Underwriting: moratorium vs full medical

Because fertility treatment is excluded outright, underwriting type mainly affects investigation cover and any related gynaecological or urological condition. Under moratorium underwriting, a condition you've had symptoms, treatment or advice for in the five years before joining is excluded until you go two continuous years without needing anything for it; under full medical underwriting, the insurer reviews your history at application and may apply a specific permanent exclusion instead (Bupa By You policy guide). See our guide to moratorium vs full medical underwriting for how each works in practice.

Corporate and group scheme IVF add-ons

Some insurers can agree a bespoke IVF benefit as part of a corporate or group health insurance scheme, as an employer-chosen addition rather than a standard product feature. Terms, cycle limits and eligibility vary by insurer and scheme, and need to be agreed and confirmed directly with the insurer at application — we always check the exact wording rather than assume it mirrors any other employer's scheme.

If you're an employer exploring this as a benefit, we recommend asking the insurer for the specific policy wording covering IVF before comparing cost, since the detail (cycle limits, age limits, waiting periods) will shape whether it meets your staff's needs.

How PremierPMI helps

We compare 10+ leading UK health insurers and explain their policy wording in plain English, including exactly where fertility investigation cover ends and the IVF exclusion begins. See our full list of UK health insurance providers we compare.

For employers asking about a group IVF add-on, we check the insurer's specific terms before recommending a scheme, and we're on hand to help with any claims questions once you're covered.

A note on medical advice

PremierPMI broker desk notes

  • This is one of the most common misunderstandings we come across. People assume comprehensive PMI will help fund IVF because it covers 'everything else' — it doesn't, on any individual policy we've reviewed. We explain this clearly before anyone buys, so there's no disappointment at claim stage.
  • Where a client has had fertility investigation done privately, we check whether the referral and tests were billed as eligible outpatient diagnostics, and we help query any claim that's been declined on fertility-exclusion grounds.
  • For employer clients asking about staff retention, we point out that any IVF benefit has to be arranged as a specific group-scheme addition with the insurer — it isn't something that can be added to an individual policy after the fact.
  • We've had clients confuse 'fertility investigation' cover with 'fertility treatment' cover. We're careful to separate these terms when we compare policy wording, because the gap between what's covered and what isn't sits exactly at that line.

Common questions

Does UK private health insurance cover IVF?

No. Individual PMI excludes IVF, ICSI, IUI and donor gametes on every major UK insurer's policy documents. Specialist-referred fertility investigation is generally covered; the treatment itself is not ([AXA Health handbook, April 2026](https://www.usaycompare.co.uk/hubfs/Axa%20Personal%20Health%20Handbook%20April%202026.pdf)).

What fertility investigation is covered by PMI?

Comprehensive policies typically fund specialist consultations, hormone blood tests, semen analysis and pelvic scans used to find the cause of fertility difficulty, provided these are referred by a specialist and the condition is not pre-existing.

When does the NHS investigate infertility?

Usually after two years of trying to conceive through regular unprotected sex, though a GP may refer sooner if the woman is 36 or over or there's a known risk factor, such as prior cancer treatment ([NHS](https://www.nhs.uk/conditions/infertility/diagnosis/)).

Can my employer add IVF to our company health insurance?

Some insurers offer IVF as an optional addition to group/corporate schemes, on terms agreed with the employer. This is not available on individual personal policies, and PremierPMI will confirm exact terms directly with the insurer before recommending it.

Is fertility treatment covered if PCOS or endometriosis is the cause?

No — the exclusion of assisted conception treatment applies regardless of the underlying cause. Investigation of the underlying condition may be covered separately; see our [PCOS](/conditions/pcos) and [endometriosis](/conditions/endometriosis) guides.

How much does private IVF cost in the UK?

Costs vary by clinic and are not something PremierPMI can quote, since it falls outside PMI cover. Speak to a licensed fertility clinic directly for a current, itemised quote covering the cycle and any medication.

What NICE guidance covers fertility investigation?

NICE's fertility guideline (NG257) sets out how investigation and semen analysis should be carried out, including reference values used by clinicians to interpret results ([NICE NG257](https://www.nice.org.uk/guidance/ng257/chapter/investigation-of-fertility-problems-and-management-strategies)).

How does PremierPMI help with fertility cover questions?

We compare 10+ leading UK health insurers' policy wording on fertility investigation, explain exactly where cover stops, and check whether a past fertility history will affect underwriting before you buy.
