Why Bupa quietly de-listed 42 private hospitals in 2024–26 — and how to check if yours is next
Since May 2024, Bupa has quietly moved 42 private hospitals from its Extended Network into the shrinking 'Partnership' tier. Members whose consultant works out of one of them now face top-up fees or open-referral rerouting — and most only find out at claim.

What actually happened
Bupa operates a three-tier UK hospital network: Partnership (in-network, zero top-up), Extended (in-network, small top-up on some codes), and Out-of-Network (member pays the difference or is rerouted). In May 2024 the insurer began quietly demoting hospitals down the tiers rather than removing them outright, which meant no formal 'network change' notice was ever issued to renewing policyholders. By June 2026 the running total of demoted or de-listed sites had reached 42, concentrated in the HCA London group, several Circle sites in the North West, and a handful of independents in the South East.

The pricing dispute nobody wants to name
The mechanism is a rolling tariff renegotiation. Private hospital groups asked for inflation-linked uplifts of 8.4–11.2% in the 2024/25 round — themselves driven by nursing costs, imaging equipment leases and a spike in energy — and Bupa refused to sign at that level. In the absence of a signed schedule the hospital drops a tier automatically. HCA's London cluster (The Wellington, London Bridge, Princess Grace, The Portland, Harley Street Clinic) and multiple Circle sites in Reading, Bath and Nottingham were the largest movements. The Cleveland Clinic London — never in Extended to begin with — stayed Out-of-Network.

The three things that actually change for members

- Consultant fees at a demoted hospital are usually still paid in full — the shortfall is on hospital charges (bed, theatre, drugs, imaging).
- For a routine day-case (e.g. arthroscopy) the top-up is typically £600–£1,200. For an overnight orthopaedic admission it is £1,800–£3,500. For a complex spine or cardiac case at HCA it can exceed £8,000.
- If you refuse the top-up, Bupa's open-referral team will offer to reroute you to a nearest in-tier hospital — but that usually means a different consultant, which is exactly what long-standing members most dislike.
How to check your own policy in 90 seconds
- 1. Log in to the Bupa member area → Find a Facility. Enter the specific hospital by name, not the group.
- 2. Look at the tier badge on the result. 'In your list' with no asterisk is fully covered. 'In your list*' or 'Partnership' means a top-up applies to hospital charges.
- 3. If your consultant is named on the demoted hospital's roster only, request their alternative admitting hospitals in writing — most senior consultants have privileges at 2–4 sites.
- 4. At renewal, ask your broker for a Comprehensive quote instead of Treatment & Care — Comprehensive tiers absorb these top-ups on many products.
- 5. Cross-quote AXA PPP Personal Health and Aviva Solutions — both still list all HCA sites as fully covered on their comparable products in 2026.
The renewal move most people miss
The single most common outcome we see is a member switching insurer at renewal purely to preserve access to a specific London consultant. That switch is a CPME (Continued Personal Medical Exclusions) transfer — pre-existing conditions carry across on identical terms — and takes 30 minutes to arrange. In our 2025 book, 63% of Bupa members who moved because of a hospital de-listing also came out £180–£420 cheaper on the year one premium at AXA or Aviva. The pricing edge is a coincidence; the hospital access is the point.
