Where Aviva wins
- Fast-track physio & mental health referral without needing a GP letter (self-referral pathway).
- GP24 video service is genuinely useful — bookable inside MyAviva, prescriptions filled digitally.
- Overseas travel cover included on most tiers up to 120 days per trip (subject to policy).
Where Bupa wins
- Cancer Promise — genuinely differentiated ongoing cancer cover.
- Bupa Health Clinics for GP-level assessments without a full consultant referral.
- Bupa Anytime HealthLine — 24/7 nurse-led triage, no policy limit on calls.
Aviva watch-outs
- Consultant fee shortfalls: Aviva pays 'reasonable and customary' — a small number of London consultants charge above this. Ask for a fee estimate before booking.
- Chronic conditions are, as with all UK PMI, excluded once diagnosed and stable — Aviva's definition is broadly in line with the ABI.
- Extended Cover Plus roughly adds 20–35% to premium in London postcodes — worth it only if you want central-London HCA access.
Bupa watch-outs
- Guided Consultant Network is materially cheaper — but you don't get open choice of consultant. Read the wording carefully.
- Bupa is typically 15–25% more expensive than Aviva at like-for-like ages and postcodes.
- Renewal increases can be sharper than mid-market insurers in years where a claim has occurred; broker retention negotiation genuinely matters here.
Hospital access compared
Aviva: Aviva runs three tiers: Key Hospital List (network only, cheapest), Directory (broadest UK network, most policies default here), and Extended Cover Plus (adds central-London HCA sites — vital if you want The Wellington, The Harley Street Clinic or The Princess Grace).
Bupa: Bupa's Guided Consultant Network (choose from a shortlist Bupa selects for your condition) delivers the sharpest pricing. Standard and Extended hospital lists progressively add more sites including HCA hospitals. Cromwell Hospital in Kensington is Bupa-owned and open-access to Bupa members.
Underwriting compared
| Insurer | Underwriting options |
|---|---|
| Aviva | Moratorium — no medical forms; conditions in the previous 5 years excluded until 2 years symptom-free.; Full Medical Underwriting (FMU) — full disclosure up front, clearest exclusions, often the fairest option if you have a stable managed condition.; Continued Personal Medical Exclusions (CPME) — used when switching from another insurer; keeps existing exclusions. |
| Bupa | Moratorium; Full Medical Underwriting; Continued Personal Medical Exclusions |
PremierPMI verdict
Aviva remains our workhorse recommendation for individual and family PMI where a client wants breadth, a solid app and predictable renewal behaviour. It's rarely the cheapest and rarely the most premium — it's the balanced middle, and that's why we place so much of it.
Bupa is the premium buy for clients whose priority is depth of cover on the claims that scare them most — cancer, complex cardiac, complex musculoskeletal — and who value direct-access clinics. You pay for it, and it's usually worth it when it matters.
- Choose Aviva if: Balanced family PMI with strong app + digital GP.
- Choose Bupa if: Best-in-class cancer cover + owned clinics.
- Both are placed whole-of-market by PremierPMI — we quote each side by side so you compare like for like before you commit.
