Aviva watch-outs
- Consultant fee shortfalls: Aviva pays 'reasonable and customary' — a small number of London consultants charge above this. Ask for a fee estimate before booking.
- Chronic conditions are, as with all UK PMI, excluded once diagnosed and stable — Aviva's definition is broadly in line with the ABI.
- Extended Cover Plus roughly adds 20–35% to premium in London postcodes — worth it only if you want central-London HCA access.
Vitality watch-outs
- If you won't engage with the app, the rewards ecosystem is dead weight — pick Aviva or AXA instead.
- Vitality's underwriting stance on some pre-existing conditions can be stricter than Aviva's — always compare the moratorium wording.
- The rewards you 'earn' require ongoing activity tracking — if you drop off, benefits reduce at anniversary.

Aviva: Aviva runs three tiers: Key Hospital List (network only, cheapest), Directory (broadest UK network, most policies default here), and Extended Cover Plus (adds central-London HCA sites — vital if you want The Wellington, The Harley Street Clinic or The Princess Grace).
Vitality: Vitality operates Countrywide (broad national list), Guided (network only, cheaper), and Advance (adds central London HCA sites). Vitality has become notably better on hospital-list breadth in recent years.
